Annual Membership Includes:
›› Unlimited doctor visits for one calendar year.
›› Quick check labs: pregnancy, strep, finger stick blood sugar, dipstick urinalysis &
hemoglobin A1C
›› EKG, spirometry, and basic x–ray services.*
›› Minor Surgery (biopsy of miles, suturing of minor lacerations)*
›› Injury evaluation
›› Treatment of common illnesses
›› PLUS!! CBC, chemistry profile, lipid panel, annual Pap smear or PSA if needed.
›› Photo ID membership card
